But for Dr. F. Ramzi Asfour, the path he set as a young boy never wavered.<\/p>\n<p>\u201cMy close friend Jay and I went to pediatricians who were partners, Drs. Kent and Errante. Dr. Kent was also a troop leader, and we looked up to him,\u201d he recalls. \u201cWe decided we would become doctors and take over their practice when they retired.\u201d<\/p>\n<p>He pauses with a smile. \u201cWell, I never varied from that decision\u2014but my friend is now a Jewish Rabbi.\u201d<\/p>\n<p>For Dr. Asfour, medicine wasn\u2019t a default choice or fallback career. It was a calling, something both noble and deeply personal. He has been a practicing clinician, a public health advocate, a mentor, and a problem solver.<\/p>\n<p>But perhaps most notably, he has seen medicine through multiple lenses: global health, private practice, rural health, big city tertiary care, telehealth innovation, outbreak management, and now as an expert witness.<\/p>\n<p>\u201cThere were three things I dreamed of when I was younger,\u201d he says. \u201cOne was working for the World Health Organization, one was having a Swiss bank account, and the other was owning a BMW. I achieved all three, though only the WHO job lived up to expectations. The bank account I got primarily because I was living in Geneva at the time and only had $2,000 in it, and the BMW was 13 years old, had a dent in the passenger door, and smelled like gasoline inside when you drove it.\u201d<\/p>\n<p>While working for WHO, he traveled across many English-speaking parts of Africa\u2014Ghana, Uganda, Botswana, Tanzania, Lesotho, Kenya, South Africa. \u201cI\u2019ve always felt a pull toward Africa,\u201d Asfour shares. \u201cI\u2019ve never quite understood why Africa resonated with me so strongly. But one thing was clear, I wanted to be an infectious diseases doctor, in part so I could go where the need was greatest. Working in Ghana, during the HIV crisis, I witnessed the daily struggle for basic medical attention\u2014it left an imprint on me.\u201d<\/p>\n<p>Back home in San Francisco, the struggles were different\u2014homelessness, substance abuse\u2014but it reaffirmed for him that the need for compassionate, competent care is universal.<\/p>\n<p>He began working to address the \u201cdeserts\u201d of care. \u201cI developed my telemedicine infectious disease practice as a way to bridge the gap for rural communities in California,\u201d he says.<\/p>\n<p>Capsid Consulting, another of his companies, helps nursing homes improve infection prevention and care for residents\u2014the impact of its work was most clearly seen during COVID. I agreed\u2014and the next thing I know, six banker\u2019s boxes of paperwork arrive.\u201d<\/p>\n<p>He pauses. \u201cI started sorting through everything\u2014papers on my desk, on the floor, everywhere. After speaking with my wife, I decided I might take on more cases, but first, I was going to hire an assistant to help organize all the documentation. That was one of the best decisions I\u2019ve ever made.\u201d<\/p>\n<p>The experience taught him two key things: first, how to methodically extract relevant facts from mountains of data; and second, that there was a real need for physicians who could not only interpret complex medical issues, but also communicate them clearly in legal contexts.<\/p>\n<p>\u201cIt was like learning a new dialect,\u201d he explains. \u201cYou have to be accurate, but also understandable. His portfolio includes criminal and civil cases, ranging from personal injury and product liability to wrongful death and medical malpractice.<\/p>\n<p>One recent case stands out \u2013 a wrongful death suit related to workplace exposure to COVID-19.<\/p>\n<p>\u201cI was flying into Boston, just about to land, when I got a text from the attorney saying, \u2018We\u2019re about to settle,\u2019\u201d he recalls. \u201cThis was a workers\u2019 compensation case in which the plaintiff claimed the decedent contracted COVID at work and died. But in reviewing the ambulance records, I noticed that when the wife made the call, she said it was her son who had COVID\u2014not her husband.\u201d<\/p>\n<p>Asfour dug deeper. The son had, in fact, called in sick to work around that time. \u201cI advised the attorney to verify those records. She did. And that new information changed the calculus. The case settled just before going to trial\u2014and I got a few nice days in Boston.\u201d<\/p>\n<p>Stories like that aren\u2019t rare in his line of work. Sometimes it\u2019s a timeline discrepancy. Sometimes it\u2019s a chart entry that contradicts a deposition. Through his volunteer faculty appointment at UCSF, he teaches infectious diseases to internal medicine residents at Highland Hospital in Oakland. Through Praxis, he coaches physicians on how to become effective expert witnesses.<\/p>\n<p>\u201cIt\u2019s not enough to know the medicine,\u201d he says. \u201cYou need to know how to prepare a report, how to explain medical concepts to a jury, how to withstand cross-examination. It\u2019s a learned skill.\u201d<\/p>\n<p>He also talks about the emotional component, especially for doctors used to \u201cfixing\u201d things. \u201cYou\u2019re not there to save a life. You\u2019re there to tell the story of what happened, from a medical perspective. And sometimes the answer is uncomfortable or unclear.
